This collection contains a register, arranged chronologically by date received, of human cadavers received by the Anatomical Board to be distributed to medical and dental facilities more for research. Information provided for each cadaver includes name, gender, color, age, place of birth, marital status, and occupation, date received, date and cause of death, physician's name, where received from and delivered to, tag number, whether cut or uncut, condition, and the claimed, substitute, or buried tag number. In addition, there is a monthly consolidated report of receipts from institutions giving the total number of bodies, the number of cut and uncut bodies, the number of white or black males and females, the number or bodies in good, bad and fair condition, the number of claimed, substitute, and buried cut and uncut bodies, and occasional remarks regarding each institution providing cadavers to the Anatomical Board. less

This collection contains a record, arranged in numerical order by application number, of individuals licensed as chiropodists from October 26, 1956 to August 14, 1974. Information more provided includes the name and address of the chiropodist; the date of graduation; the name and location of the institution which granted the degree; the date of application and the application number; the date that the license was issued and the license number; and the date that the license fee was paid and the amount. There is space available for recording the person's age and date of examination but it is left blank. As needed, a remarks section was used to indicate the date that the chiropodist died. Images of reports of inquisitions conducted by a County Coroner and a twelve man panel who had viewed a body attesting to the cause of death. more These inquests determined the need for criminal prosecutions, and were used as the basis for conviction at the grand jury inquest if needed. Information provided includes name of decedent; date and cause of death; date of inquest; and county in which it took place. Causes of death listed include criminal acts (assault, murder); medical problems (poison, drinking water, apoplexy, flu, burns, fits, palsey, opium overdoses, and suffocation); and accidents (drowning, being hit by falling trees and logs, sleepwalking out a window, falling from buildings, horses, ladders, and chairs, or into wells, and being run over by wagons or horses, or both). Other causes listed are suicide, natural causes, and "upon the visitation of God." Places where inquests were conducted include Berks, Bucks, Chester, Cumberland, Delaware, Northampton, and Northumberland Counties, and Philadelphia. Other documents sometimes included with the inquisitions are Sheriff's Inquests and Grand Jury Inquests for Oyer and Terminer in Bucks County, death verifications, and witness depositions. This collection includes: certificates signed by the examiners who reported the licentiate as having passed the dental examination. The certificates give the name of the dentist, more the date and place where the examination was passed, the date the license was issued, and the agency and officials conferring the license which are part of series 22.41 and 22.95. Images of investigations into health hazards and pollution complaints at various industrial plants throughout the state are included in this series. Records span numerous years for more some facilities, such as a thirty-year run of data for the Lebanon Steel Foundry. Information includes: data on medical problems for individuals caused by hazardous working conditions (with names of the individuals given in a few instances), specifics on the types on machinery utilized at specific plants, employment statistics, survey records, inspection results and related laboratory reports, as well as descriptions of miscellaneous industrial processes. less

This collection contains a record, arranged alphabetically by surname of chiropodist, of licenses issued to chiropodists by the State Board of Chiropody Examiners between May 12, more 1914, and March 15, 1956. Entries give the name, address, and license number of the practitioner and the date that the license was issued. Information such as whether the individual died or had a name change is sometimes noted too. This collection contains a record, arranged chronologically, and thereunder numerically by license number, of licenses issued on the basis of the applicant having already been accepted more as a qualified physician in another state. Entries list the name of the physician, the date licensed, the out of state examining board and license number, and the agency conferring the license. Some certificates are stamped with the official seal of the Medical Council. Also, some certificates note which state had previously licensed the physician. The more certificates for which the State Medical Society was the examining board list the name of the physician; the date and place where the examination was passed; the agency conferring the license; and the date the license was issued. Many of the licenses are subscribed by officers of the Medical Council and the physicians who reported the licentiate as having passed the examination. This collection, arranged chronologically by the date the license was issued, contains copies of licenses issued to optometrists by the State Board of Optometrical Education, Examination, more and Licensure. Pertinent information found includes the name and address (city, state, and county) of the optometrist, the date of licensing, and in some instances comments about whether the licensee died or had his or her certificate revoked. less

This collection, arranged numerically by certificate number and indexed internally, alphabetically by the first letter of the applicant's surname, contains a record of the persons granted more teaching certificates by the State Board of Education. Entries for the 1868-1896 volume include the recipient's name, post office, and county of residence; the date the certificate was issued; the Pennsylvania School Journal volume and page relating to the recipient's certification; and oftentimes, a description of the subjects studied. This collection has entries prior to 1913, arranged in strict alphabetical order but after 1913 they are only grouped alphabetically by surnames. A record of physicians more who were registered in Pennsylvania from 1870 to 1917. Normally, the doctor's name, license number (from 1895 onward), postal address, county where registered, and date of registration are given. Oftentimes information concerning the death or change of address of the practitioner is included. Note: Volume H-M are not part of the physical holdings and as such are not available digitally. less

This collection contains meeting minutes, arranged chronologically by date of meeting, of the Sanitary Water Board. Information includes time, date and location of the meeting, a more list of the names of members and staff present, a synopsis of discussions, and the signature of the Secretary of the Board. Also present are reports from various agencies, including the Department of Health, Department of Mines, Fish Commission, Department of Forests and Waters, Department of Agriculture, and the Department of Commerce. The minutes document the approval of permits for sewage and industrial waste treatment and mine drainage discharges. The volume for 1923 contains an index. less
