Act 48 Form for POWER Pop-Ups

Please complete this form for Act 48 credit for attending a POWER Library Pop-Up. We will submit the Act 48 requests upon the completion of all Pop-Up sessions, and then it typically takes 8-10 weeks for Act 48 credit to appear in the Professional Education Record Management System.
