Denial of Consent for Minor to Obtain an E-Card

A minor in your household listed your email as the adult/guardian email when they subscribed for an E-Card with POWER Library. An E-Card with POWER Library provides access to the e-resources (databases) available on the following webpages:

The e-resources and e-book collections serves to support and supplement the educational needs of the K-12 community as well as the general educational interest, recreational, and entertainment needs of the public, and reflects the racial, ethnic, and cultural diversity of the Commonwealth.

If you do NOT want the minor in your household to have an E-Card to access to the e-resources (databases) in POWER Library, please submit this form. Please note that it may take 2-4 business days for the E-Card number to be removed from our system.
